Abstract
Summary: Evaluation of Tumor Necrosis Factor α Concentration in Body Fluids during Local and Systemic Exposure to Endotoxins Tumor‐necrosis factor α (TNF) has been shown to have a pivotal role in mediating biological effects of endotoxins. Therefore, TNF measurement in the circulation is thought to be a useful diagnostic and prognostic instrument in patients exposed to endotoxin. However, the character of the TNF response depends on the mode of endotoxin exposure and is influenced by a variety of factors. Furthermore, to interpret the TNF state, the regulative effects of the two soluble TNF receptors have to be considered. Finally, the current techniques of TNF determination have to be improved and standardized to obtain reliable and comparable results and to establish TNF measurement as a valuable laboratory parameter. Zusammenfassung: Der Tumor‐Nekrose‐Faktor α (TNF) spielt eine zentrale Rolle bei der Vermittlung der biologischen Wirkungen von Endotoxinen. Deshalb wird die TNF‐Messung in der Zirkulation als nützliches diagnostisches und prognostisches Instrument bei Patienten mit einer Endotoxinexposition angesehen. Der Charakter der TNF‐Antwort ist jedoch von der Art der Endotoxineinwirkung abhängig und wird durch eine Vielzahl von Faktoren beeinflußt. Weiterhin müssen bei der Interpretation des TNF‐Status die regulatorischen Effekte der beiden löslichen TNF‐Rezeptoren berücksichtigt werden. Letztlich bedürfen die gegenwärtigen Techniken der TNF‐Bestimmung einer Weiterentwicklung und Standardisierung, um verläßliche und vergleichbare Ergebnisse erzielen und die TNF‐Messung als aussagekräftigen Laborparameter etablieren zu können.
